Job Summary:
Responsible for the initial orientation, ongoing education, and support for clinical and non-clinical staff. Responsible for developing and implementing a comprehensive clinical informatics education program. Coordinates and collaborates with multiple disciplines and clinicians to ensure EMR meets all regulatory requirements. Collaborates with Managers and Staff to insure most efficient documentation system within EMR. Provides problem solving and trouble shooting for staff and managers. Liaisons with EMR vendor and CNE IS. Is responsible to the Administrator for the management of professional services, the standards of practice and other related professional aspects of patient care services.
Duties and Responsibilities:

Utilizes the Care New England standard of ACT values as a basis for delivery of care and/or services
Coordinates agency wide orientation training for all clinical and office staff.
Coordinates and arranges patient assignments, during orientation
Assesses initial competency as relates to practice area, patient clinical needs, and regulatory requirements for clinical documentation.
Initiates and completes clinical skills assessment and orientation checklist.
Collaborates with manager to complete the probationary evaluation for all clinical and non-clinical staff
Evaluates the effectiveness of orientation in relationship to specific measures
Conducts competency assessments as needed
Proactively identifies staff learning needs through chart reviews and direct observation during office and home visits
Prepares, presents and evaluates the effectiveness of in-services and educational sessions
Develops and manages a master education calendar. Presents proposed education and summary of previous events at appropriate committee
Develops and coordinates agency wide annual competency fairs
Assists in securing resources for specialty education needs
Participates on agency, hospital and external committees.
Identifies and assists in providing opportunities to develop/expand clinical expertise of staff and maintain timely competencies.
Acts as a resource for clinical staff.
Collaborates with the leadership team provide sound and relevant educational experiences for staff.
Provides continuity of care by assisting with linking patient needs with the appropriately skilled clinician.
Educate clinicians on case management practices to ensure appropriate utilization, consistency in visits and high-quality outcomes.
Participates in and reports departmental quality indicators to the VNA Quality Improvement Committee.
Evaluates the effectiveness of educational sessions. Reports results and uses data to make changes
Collaborates in providing education related to TJC, regulatory requirements
Support and provides education on new clinical initiatives (ex cardiac care, wound etc)
Works with managers to develop and implement and evaluate PIP or staff who need additional assistance
Develops and reviews policies/procedures as required.
Involves managers and other staff as necessary regarding the aspects of planning and implementing software changes
Integrates all training on computer systems and documentation within the context of staff responsibilities
Provides orientation and ongoing training to all clinical and non-clinical staff regarding EMR and other platforms
Provides training/support for staff as it relates to technology
Creates customized process/screens for the use by specialties and to meet documentation requirements
Participates in organizational meetings and training venues as requested.
Serves on appropriate agency committees.
Serves as a preceptor or peer guide for students or new employees as requested.
Requirements:
Bachelor’s degree in nursing required. Master’s degree in Specialty area preferred.
(5) years’ experience in a hospice/home care environment required. A minimum of 3 years in home care preferred. Prior experience in clinical and computer teaching roles preferred.
Current RN licensure in State of Rhode Island required.
Must possess a valid driver's license and current automobile liability insurance.
Must be BLS/CPR certified.
Oasis Certification preferred, CHPN preferred.
