Job Summary:

The Providence Center Substance Use Specialist ACT Team will coordinate services for clients to minimize stressors and to facilitate substance abuse and mental health treatment.

Duties and Responsibilities:

Coordinate a variety of services.

Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with substance abusers, as it relates to case management.

Present verbally a synopsis of actual cases as required.

Accompany clients to appointments with agencies, clinics, doctors, etc.

Initiate vocational/job development activities with clients.

Monitor client status during the spend-down process, complete necessary DHS applications

Provide individual mental health and/or substance abuse counseling/therapy.

Perform in a calm and professional manner during crisis or emergency situations.

Complete substance abuse assessments on all clients.

Identify substance abuse treatment interventions with staff and clients.

Maintain confidentiality in accordance with center policy and legal requirements.

Develop and implement a plan of care with ongoing client input.

Apply knowledge of Psychiatric Diagnosis (DMS IV).

Apply knowledge of substance abuse and primary methodologies of substance abuse treatment.

Assess clinical emergencies, including suicide and danger to others.

Develop and present, verbally and in writing, a clear clinical formulation based on behavioral data and relevant theory while incorporating psychosocial and family issues.

Provide specialty substance abuse services to clients and serve as a resource to staff regarding issues relative to substance abuse treatment.

Make assessments and provide appropriate crisis interventions and risk management.

Teach clients adult daily living skills.

Assist clients with benefits acquisition (social security supplemental insurance (SSI), food stamps, advocates office, medical doctor).

Complete required interagency and other necessary paperwork.

Negotiate on behalf of the client, as needed, with landlords, courts, families, the treatment team, and consumers themselves.

Transport clients in own vehicle to planned activities and appointments.

Complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by TPC confidentiality policy.

Maintain cooperative relationships with Center staff, clients, community agencies and the public.

Attend trainings, case presentations and conferences.

Serve on appropriate Center committees.

Requirements:

Bachelor’s degree in social work of Human Services or related field.

Two years of experience in substance abuse therapy, as well as experience dealing with the dually diagnosed. Must have awareness of the general trends in confronting the problems of substance abuse.

CDP/LCDP a plus.

Must have valid driver's license and insured auto.

Bilingual Spanish candidates preferred.
