Job Summary:

Home Base is a partnership of The Providence Center (Home Base ACT Team), Rhode Island housing providers, homeless advocates, and community organizations designed to combat chronic homelessness in the state. Home Base aims to support chronically homeless individuals who have a co-occurring mental health and substance abuse disorder by assisting them in securing permanent housing and obtaining treatment that will help them maintain long-term recovery.

The Providence Center Home Base ACT Team Case Manager will join the Assertive Community Treatment (ACT) Team, with a focus on providing additional homeless and housing services for dually-diagnosed adults in the Providence and surrounding areas.

Duties and Responsibilities:

Coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms.

Provide supportive counseling, problem solving, contracts and limit setting.

Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with substance abusers, as it relates to case management.

Arrange home visits for consumers.

Accompany consumers to appointments with agencies, clinics, doctors, etc.

Initiate vocational/job development activities with consumer.

Make assessments and provide appropriate crisis interventions and risk management as needed.

Assess consumers' unmet needs (psychological, substance use, physical, housing, psychiatric).

Identify, assess and respond to crisis situations in a calm and professional manner.

Teach consumers adult daily living skills. Teach, supervise and assist consumers with doing household chores, laundry, room cleaning, etc.) using realistic situations.

Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.

Advocate for consumer.

Transport consumers in own vehicle to coordinated activities and appointments.

Complete progress notes with appropriate format.

Develop and implement a plan of care with ongoing consumer input.

Maintain cooperative relationships with Center staff, consumers, community agencies and the public, and work in cooperation with co-workers and supervisory staff. -

Attend trainings, case presentations and conferences.

Other duties as assigned.

Requirements:

Associate's degree required. Bachelor's degree from four-year College or University in Human Services preferred.

Experience in Human Services field preferred.

Experience serving homeless individuals and experience with public housing resources is a plus.

Must have valid driver's license and insured auto.
