Position Overview: We are seeking a dynamic Nurse Practitioner/Physician Assistant to join our Integra at Home program. The successful candidate will be instrumental in delivering excellent, patient-focused healthcare by actively engaging with our diverse team of medical experts. This role involves utilizing a combination of face-to-face interactions, utilizing telehealth platforms and remote monitoring technologies to deliver tailored and proactive healthcare services.

Job Responsibilities:

  1. Conduct comprehensive assessments, diagnose medical conditions, and develop individualized treatment plans for patients within the Integra at Home program.
  2. Perform physical examinations and order diagnostic tests as necessary to evaluate patients' health status and monitor their progress over time.
  3. Prescribe medications, therapies, and other treatment modalities in accordance with established protocols and clinical guidelines.
  4. Collaborate with physicians, nurses, social workers, and other healthcare team members to coordinate care and ensure continuity of services for patients.
  5. Provide patient education and counseling on disease prevention, treatment options, medication management, and lifestyle modifications to promote optimal health outcomes.
  6. Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery and outcomes.
  7. Maintain accurate and up-to-date medical records, documentation, and billing codes in compliance with regulatory standards and organizational policies.
  8. Demonstrate a commitment to professional growth and development by actively pursuing continuing education opportunities, and staying abreast of current evidence-based practices.

Qualifications:

  1. Master's degree in Nursing (MSN) or Physician Assistant Studies (MPAS) from an accredited program.
  2. Minimum of 1 year of clinical experience in a relevant healthcare setting, preferably in primary care, internal medicine, or a specialty area related to complex medical conditions.
  3. Proficiency in conducting comprehensive health assessments, diagnosing acute and chronic illnesses, and managing complex medical cases independently.
  4. Strong interpersonal skills with the ability to communicate effectively and collaborate with patients, families, and members of the healthcare team.
  5. Demonstrated commitment to providing compassionate, patient-centered care and promoting health equity and cultural competence.
  6. Ability to adapt to a fast-paced work environment, prioritize tasks efficiently, and demonstrate flexibility in responding to changing patient needs and organizational priorities.

Preferred Qualifications:

  1. Experience working in an integrated healthcare delivery system or accountable care organization (ACO) model.
  2. Certification or specialized training in a relevant area such as geriatrics, palliative care, chronic disease management, or behavioral health.
