The Case Manager and Residential Case Manager positions are eligible for $2500 sign on bonus and prorated for part time positions.

Job Summary:

Provide clinical case management services in order to facilitate program goals and objectives using a quality care approach.

Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders.

Working with consumers to achieve the highest level of independence possible.
