Nurse Practitioner/Physician Assistant – Home Care /Telehealth – Integra
Care New England, a fully integrated health system serving residents throughout Rhode Island is seeking a Nurse Practitioner or Physician Assistant provide care management for our Geriatric patients with Integra. Integra is a community of doctors, nurses, social workers, pharmacists, community health workers, and patients working together to improve the health and well-being of our community.
Position Highlights

- Average Patient Age: 60+
- Monday – Friday; 8:00am – 4:30pm schedule with rotational paid call
Job Responsibilities:
- Conduct comprehensive assessments, diagnose medical conditions, and develop individualized treatment plans for patients within the Integra at Home program.
- Perform physical examinations and order diagnostic tests as necessary to evaluate patients' health status and monitor their progress over time.
- Prescribe medications, therapies, and other treatment modalities in accordance with established protocols and clinical guidelines.
- Collaborate with physicians, nurses, social workers, and other healthcare team members to coordinate care and ensure continuity of services for patients.
- Provide patient education and counseling on disease prevention, treatment options, medication management, and lifestyle modifications to promote optimal health outcomes.
- Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery and outcomes.
- Maintain accurate and up-to-date medical records, documentation, and billing codes in compliance with regulatory standards and organizational policies.
- Demonstrate a commitment to professional growth and development by actively pursuing continuing education opportunities, and staying abreast of current evidence-based practices.
