Job Summary:

The Lung Nodule Clinic Patient Navigator is responsible for coordinating the lung nodule clinic and all aspects of the patient journey within Care New England and its affiliated care partners. Patient Navigators are individuals with excellent communication and interpersonal skills who establish trusting professional relationships with patients. Under the general direction of the office manager or other licensed clinical team member, the Patient Navigator provides patient and practice support to enhance patient engagement in meeting care plan goals. The incumbent works with the practice to close care gaps and works closely with primary care teams by identifying and removing barriers to care.

Duties and Responsibilities: 

Serve as the primary point of contact for patients and families navigating the lung nodule clinic care process.

Provide administrative and clinical coordination support for CNE physicians, advanced practice providers, schedulers, billing staff, and care teams involved in the lung nodule clinic care.

Collaborate closely with office staff to schedule and track patients across the CNE system.

Pre-clinic:

Obtain relevant medical records, imaging CDs, and reports from both CNE and outside facilities.

Ensure all preparation materials are collected and reviewed prior to clinic appointments.

Verify insurance coverage and out-of-network benefits when applicable.

Confirm patient scheduling aligns with clinical and administrative expectations.

Share patient lists with billing staff for coordination of services.

During clinic:

Provide patients with information about their care plan, appointment logistics, and expectations.

Assist patients with administrative needs such as registration or paperwork.

Notify relevant providers of confirmed visits and procedures.

Post-clinic:

Distribute and upload clinic documentation into the medical record.

Ensure timely transfer of records to other providers and facilities as needed.

Maintain a registry of treated patients for clinical tracking and quality assurance.

Collaborate with research staff when patients are eligible for clinical trials.

Monitor patient events and support compliance with follow-up protocols, including:

Scheduling and confirming post-operative or follow-up visits.

Tracking 30-day and 1-year follow-up appointments.

Arranging local imaging or testing (e.g., CT scans, echos, COVID tests).

Reviewing results and routing them appropriately.

Coordinating care for patients receiving services at outside facilities.

Maintaining up-to-date records on testing dates and readiness for follow-up.

Obtain authorization for procedures

Perform all other related duties as assigned.

Requirements:

High school diploma or GED required; bachelor's degree preferred.

2-4 years of experience in a healthcare environment. Clinical experience preferred.
