This position is for the newly graduated registered nurse with less than 12 months of nursing experience. Under the supervision of the nurse residency program manager, the home health nurse resident acquires the skills and knowledge to provide direct patient care and effectively manage a caseload of patients in the home care setting. The program provides an individually paced supervised clinical experience with progressive independence and structured education in a variety of formats for adult learners. Examples include traditional classroom didactic, interactive case studies, role playing and simulation opportunities, skills labs and daily report and debriefing. The nurse resident will transition to a nurse case manager who effectively collaborates with an interprofessional team, monitors and adjusts treatments and identifies opportunities for process improvement. These responsibilities will be carried out in accordance with organization policies/procedures and standards consistent with federal, state, and accreditation regulations.
