Job Summary:

The Providence Center Case Manager BHOP Team Z2 will provide case management and substance abuse treatment to consumers in the Behavioral Health Outpatient Program (BHOP).

Duties and Responsibilities:

Coordinator a variety of services.

Make assessments and provide appropriate crisis interventions and risk management as needed.

Teach consumers adult daily living skills.

Initiate treatment and advocate for consumer within network of care.

Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.

Transport consumers in own vehicle or Center van to planned activities and appointments.

Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.

Maintain cooperative relationships with Center staff, consumers, community agencies and the public.

Attend trainings, case presentations and conferences.

Requirements:

Associate's degree in Human Services or related field required. Bachelor's degree preferred.

Experience in cross-cultural education, training, and consultation required.

Experience with Rhode Island substance abuse system and its network agencies.

Awareness of the general trends in confronting the problems of substance abuse.

Bilingual in Spanish preferred.

Must have valid driver's license and insured auto.
