SUMMARY
Coordinate services for patients to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage patients to attain the highest possible levels of independence.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Coordinate a variety of services.
-Treat patients with respect.
-Provide support counseling, problem solving, contracts and limit setting.
-Establish attainable goals.
-Involve patients, agencies and others in planning.
-Assist patients in deciding which areas they want to address.
-Encourage patients to attain the highest possible levels of independence.
-Accept the values and lifestyles of patients.
-Use a flexible approach with a diverse population.
-Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with substance abusers, as it relates to case management.
-Present verbally a synopsis of actual cases as required.
-Accompany patients to appointments with agencies, clinics, doctors, etc. (NA in current COVID environment)
-Initiate vocational/job development activities with patients.

Make assessments and provide appropriate crisis interventions and risk management as needed.
-Treat patients with respect.
-Assess unmet needs (psychological, substance use, physical, housing, psychiatric).
-Identify, assess and respond to crisis situations.
-Use a flexible approach with a diverse population.
-Perform in a calm and professional manner during crisis or emergency situations.

Teach consumers adult daily living skills.
-Use a step-by-step process when teaching skills.
-Use positive reinforcement and encouragement.
-Integrate skills training in all aspects of Rehab. and Recovery Plan.
-Maintain a clean, healthy and safe environment at all times.
-Use a flexible approach with a diverse population.
-Assist with budgeting and money management.
-Help form a nutritional plan

Initiate treatment and advocate for patients within network of care.
-Collaborate care with external providers.
-Assist with benefits acquisition (social security supplemental insurance (SSI), food stamps, advocates office, medical doctor).
-Complete required interagency and other necessary paperwork.
-Assess housing needs, and assist with housing application

Negotiate on behalf of the patient, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
-Negotiate with reasonable goals.
-Clarify the issues to be negotiated.
-Follow through with the agreement reached.
-Remain flexible and consider alternatives.
-Advocate for patient.

Transport consumers in own vehicle to planned activities and appointments. (NA in current environment)
-Drive carefully and abide by traffic rules.
-Ensure that seat belts are worn by all traveling in Center or employee owned vehicles.

Complete documentation in accordance to the hospital and licensing requirments.
-Complete daily activity logs.
-Complete progress notes with appropriate format.
-Develop and implement a plan of care with ongoing patient input.
-Complete a clear, specific, behaviorally oriented treatment plan as required.
-Monitor confidentiality in accordance with hospital policy and legal requirements.
-Complete required reports and evaluations
-Follow required office procedures in a cooperative manner.
-Initiate and process incident reports as per hospital guidelines.
-Adhere to Infection Control protocols.

Maintain cooperative relationships with patients, staff and other community providers.
-Communicate with various local agencies and other departments
-Work in cooperation with co-workers and supervisory staff.
-Deal with team members regarding problems in a constructive manner.
-Accept and use direction and supervision.
-Benefit from constructive criticism.

Attend training, case presentations and conferences.
-Provide feedback to staff on the content of educational programs attended.
-Attend mandatory in-service training and other required training.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Bachelor’s degree from four-year College or university in Human Services. Experience in Human Services Field preferred.

LANGUAGE SKILLS

Ability to read and interpret documents such as policy and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before team members and other employees and have the capacity to work with other community agencies.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers.

REASONING ABILITY

Ability to define problems, collect data, establish facts, and draw valid conclusions.


CERTIFICATES, LICENSES, REGISTRATIONS
Must have valid driver's license and insured auto.

OTHER SKILLS AND ABILITIES
Must be able to be flexible.
