Butler Hospital- Case Manager

Department: Health Path

Primary Function

Coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric symptoms, coordinate all health care needs and help individuals towards overall wellness.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Bachelor’s degree in Social Work, Human Services or related field required. At least 3 years of experience working in the field of mental health/substance use preferred. LCDP certificate preferred or if you are near completing.

CERTIFICATES, LICENSES, REGISTRATIONS
Must have valid driver's license and insured automobile.

OTHER SKILLS AND ABILITIES
Must be able to work flexible nights and weekends and or Holidays.

While performing the duties of this job, the employee occasionally works in outside weather conditions and is regularly exposed to fumes or airborne particles. Schedule: 40/80 Full Time Days Monday-Friday
