Job Summary:

The Care Transition Specialist plays a pivotal role in supporting patients through transitions of care by facilitating timely follow-up with primary care providers and conducting non-licensed medication reconciliation. This hybrid position leverages the expertise of both Pharmacy Technicians and Medical Assistants to promote continuity of care, reduce hospital readmissions, and improve patient outcomes. The Specialist also identifies individuals who may benefit from additional support and coordinates referrals to case management services.

Duties and Responsibilities:

Initiate outreach calls to patients recently discharged from hospitals, emergency departments, or urgent care facilities /settings

Confirm and schedule follow-up appointments with primary care providers.

Conduct non-licensed medication reviews by reconciling discharge medication lists with patient-reported regimens

Identify medication discrepancies, duplications, or adherence issues and escalate concerns to licensed providers or pharmacists

Assess patient needs during outreach and refer to appropriate case management programs when indicated

Reinforce understanding of discharge instructions, medication regimens, and the importance of follow-up care

Accurately document all patient interactions, assessments, and referrals in the electronic health record (EHR)

Collaborate with physicians, nurses, pharmacists, and case managers to ensure seamless care transitions

Participate in team meetings and quality improvement initiatives related to transitional care

Requirements:

Minimum one to three years experience.

High School or GED Required.

Completion of an accredited Pharmacy Technician or Medical Assistant Program.

In a healthcare setting.

Experience in care coordinator, transitional care, or patient navigation preferred.

Familiarity with case management programs and referral protocols preferred.Bilingual preferred.

Active certification/registration as a Pharmacy Technician (CPhT) or Medical Assistant (CMA/RMA), in accordance with state regulations.
