Job Summary:
The Care New England Senior Record Analyst will reanalyze patient charts for chart deficiencies, verify that Record Analysts are accurately identifying incomplete reports needed to reflect the care and treatment given during a hospital visit.
Responsible for assisting Physicians by contacting them and ensuring records are made available.
Monitors and coordinates activities in this section with the Chart Completion Supervisor.
Duties and Responsibilities:
Creates reports for Medical Staff Director and ED Director.
Creates weekly reports identifying status of un-coded records. Works closely with coding department, including daily interaction with staff and management.
Runs Dolbey and 3M reports and updates these applications.
Reports status of searches for un-coded records to HIM Management on a daily basis.
Re-Analyzes patient records for updating chart deficiencies after each chart assignment, documenting and updating the chart status in the tracking system accurately and in a timely manner.
Coordinates suspension list procedures with the Supervisor of Chart Completion.
Places telephone calls to physicians to remind them to complete their records
Orients and trains physicians in electronic signature and in all areas of chart completion.
Serves as a resource for other staff and is considered an expert in the chart completion area.
Locates patient records and documents from various file locations and storage media.
Resources efficiently and in a timely manner, providing information within established turn-around time frames.
Serves as backup for Supervisor in her absence.
Performs other related duties as assigned
Requirements:
High school degree required with three to five years’ experience in a healthcare environment required, Associates Degree preferred.
Must have knowledge of medical terminology and computer applications.
Knowledge/experience in health information applications, systems, chart tracking and release of Information regulatory requirements preferred.
Ability to work without constant supervision.
Must have excellent interpersonal and communication skills and be proficient in the English language.
