Job Summary:
The Providence Center (TPC) Outreach Clinician identifies individuals that have mental health and/or substance use needs and provides linkages to address social determinants of health. The goal of The Providence Center Outreach Clinician is to reduce the number of people experiencing homelessness/housing crises in Rhode Island due to untreated mental illness, substance use and/or co-occurring disorder through connection to treatment at The Providence Center and CCBHC services.
Duties and Responsibilities:
Build relationships with local agencies and providers to ensure a smooth referral process for those identified for TPC enrollment.
Collaborate with Case Managers and local shelters to develop and build rapport with residents and housing staff.
Work collaboratively with outpatient teams in identifying gaps in care/referrals to services needed.
Screen individuals identified as homeless or at risk for eviction for social determinants of health, behavioral health needs, housing needs and provide linkages to appropriate behavioral health, substance use, and housing services.
Participate in care coordination or service delivery planning for enrollees that focuses on stability across services and housing transitions.
Ensure that warm handoffs are made to community providers based on participants’ willingness to engage in treatment.
Collaborate with other service providers through case conferencing as needed in support of our common mission to provider referrals for individuals in need of behavioral health and/or housing services.
Ability to identify social determinants of health and coordinate appropriate referrals as needed (i.e. state and local benefits) such as Medicaid, Supplemental Nutrition Assistance Program (SNAP), disability benefits, veterans’ benefits, etc.
Connect individuals with TPC’s Housing Stabilization team for additional housing support and provide brief therapy interventions, such as motivational interviewing and goal setting.
Complete intakes and treatment plans for Housing Stabilization services.
Provide Housing Stabilization services to participants.
Attend Housing Stabilization team meetings to discuss workflow, targets, and billing.
Complete CCBHC clinical intakes and crisis evaluations for housing, as needed.
Work collaboratively with TPC’s Emergency Services department to respond to community calls regarding housing insecurity concerns and connect individuals with CCBHC services.
Provide assessment evaluations, short-term counseling, and crisis intervention as needed.
Monitor and evaluate program participant progress through internal tracking system.
Obtain and maintain active HMIS license.
Accurately collect data for HMIS, including documentation of chronic homelessness; accurately collect and enter data into the Coordinated Entry System (CES).
Maintain client and activity records related to all outreach activities.
Adhere to Housing First model utilizing motivational interviewing, trauma-informed care, and harm reduction practices that are culturally competent, client-centered, and evidence-based.
Maintain timely, accurate records, documentation, and reports as required
Attend and participate in housing-related advisory committees.
Represent The Providence Center in the community and with external partners and champion our mission.
Present a neat appearance in proper attire and identification as required by the position, department, and agency policies and procedures.
Serve on appropriate Center committees.
Attend meetings and report committee activities to staff.
Other duties may be assigned.
Requirements:
Master’s Degree in the human service field or social work or counseling preferred.
Previous experience supporting individuals with behavioral health issues and/or the homeless/housing insecure population.
Knowledge of housing barriers/resources available to individuals.
Ability to read and interpret documents such as policy and procedure manuals. Ability to write routine reports and correspondence.
Ability to speak effectively before team members and other employees and have the capacity to work with other community agencies.
Knowledge of psychosocial rehabilitation, psychotropic medication and case management.
Strong program development skills.
Demonstrate ability to work both independently and as an effective team member.
Ability to work with diverse populations.
Must have valid driver's license, registration, current inspection and insured automobile.
