Residential Psychiatric Nurse $5,000 sign on bonus

This position is eligible for $5,000 sign-on bonus.

Job Summary:

Work within a multi-disciplinary team for Residential programs to develop and implement plans of care that support and maximize the consumer’s recovery, medical care, rehabilitation, and community integration.

Duties and Responsibilities: Include the following. Other duties may be assigned.

Assess and evaluate patient health care data collected through interviewing, observation, record review, and peer and team input in order to use sound clinical judgment in the treatment planning process.

Analyze assessment data to determine a nursing diagnosis and to formulate the basis of nursing care.

Identify expected outcomes of care/treatment planning, individualized to the consumer, with the ultimate goal of influencing and improving the consumer’s overall health status.

Implement prescribed interventions. Work within a multi-disciplinary team to coordinate and/or to deliver services as prescribed, to communicate client progress and status, to manage client crises, and to actively participate in the day-to-day operations and functioning of the team.

Evaluate consumer progress in attaining expected outcomes in an on-going manner, modifying the treatment plan as indicated to reflect changes in status, needs, goals, etc.

Demonstrates knowledge of current issues and best practices of intervention and support strategies related to mental health. Develop a collaborative network within and across the service systems.

Work within a multi-disciplinary team to insure a client-centered, holistic approach to client care and treatment. To include substance abuse, medical/physical health, employment, community integration and psychotherapeutic treatment modalities.

Facilitate clinical meetings with Residential Managers and team psychiatrist to review client psychiatry appointments and any clinical concerns/supports needed. Take the lead with discussing medication concerns with psychiatrist and team.

Attend house meetings on a regular basis.

Follow up with client/clinical team on any concerns regarding medication compliance to provide education/support needed.

Address any potential substance use concern with clinical team and clients to ensure supports are identified.

Assist with incident reporting- specifically surrounding clinical concerns, medication compliance, or medication error concerns.

Educate clients/staff about medical/clinical concerns and how to best support our population using a team approach.

Work closely with Residential Managers on infectious conditions/isolation practices, etc. to ensure appropriate protocol is initiated and followed through.

Perform routine reviews of each residential medication cart/medication book to ensure medications are properly stored, handled, and ordered.

Meet Center and Team standards for the timely and accurate completion and submission of paperwork associated with the maintenance of client charts and in accordance with BHDDH and CARF regulations.

Treat consumers with respect; promote their dignity, accept their values and lifestyles and maintain their confidentiality in accordance with Center policy and legal requirements.

Provide training to staff on proper medication pass procedures

Will assist in training new nurse hires

Will conduct audits of the medication cart as well as the medication log and controlled medication log

Meet program/position requirements for scheduling and billing.

Perform record-keeping in accordance with Health Information Services and other Center and BHDDH and CARF requirements.

Complete required assessment forms.

Complete progress notes with appropriate format.

Complete required reports and evaluations.

Follow required office procedures in a cooperative manner.

Attend trainings, case presentations and conferences.

Maintain cooperative relationships with Center staff, consumers, community agencies and the public.

Communicate with various local agencies and other departments or teams at TPC.

Work in cooperation with co-workers and supervisory staff.

Deal with team members regarding problems in a constructive manner.

Accept and use direction and supervision.

Benefit from constructive criticism.

Others duties as assigned when a need is identified.

Serve on appropriate TPC committees

Requirements:

RN license required.

Bachelor's degree in Nursing preferred.

Experience working with psychiatrically disabled patients.

Must have valid driver's license and insured auto.
